Is there a family leave policy for graduate students? If so, describe.
We do not have a university policy on family leave for faculty or for graduate stuidents, but it is left to the department to decide. University policy is currently being discussed.
Is there family health insurance for graduate students? If so, is it included in the stipend?
Yes, there is, but not all of it included in the standard graduate stipend; students pay part of it (about a third). The Physics Department pays for TAs. RAs are left to the research directors to decide.
Do you have a Woman in Physics Group? If yes, please include link to their website if they have one.

Do you have a Physics Graduate Student Association? If yes, please include link to their website if they have one.
Yes, we do. The website is at http://dos.orgsync.com/org/pgsa, but it is not up to date.

We have a chapter of SPS, the Society for Physics students. Some of the members are graduate students.
Has your institution ever hosted a Conference for Undergraduate Women in Physics? If yes, which year?
Yes, in 2015.
Has your institution ever hosted an APS Climate Site Visit for Women and Minorities in Physics?
